CWC Appointments

 

How to Schedule an Appointment at the CWC

There are three ways to do so:

  • • VISIT US: Come to the CWC and ask for an IPAD at the front desk

  • View CWC Office Hours and Location

  • • SCHEDULE ONLINE: Click the button below to begin filling the required forms and book your appointment


  • • SCAN QR CODE: You may also scan the QR code below to begin the process

  • Schedule CWC Appointment QR Code

Complete the five forms listed below:

    • Triage Intake Form

    • CCAPS 62

    • Consent for Treatment

    • Consent & HIPAA

    • Availability

Once the forms are fully completed, we will be in touch with you as soon as possible to set up an appointment. Please do not leave any fields blank.

Please note: You MUST BE AN ACTIVE GOVSTATE STUDENT to receive these services. If you are not a GOVSTATE Student, please call the Counseling Lab, which services the community at 708-534-4545.
